Share this Prescription form with friends, family, and have them complete and send it back to our office. You MAY save a life or at least give them peace of mind.
- ALL 4 tests cost less than $300. *IF ANY of your test results are ABNORMAL, schedule IV Chelation at our office +cardiology.
- YOU WILL NEED TO PROVIDE our office with a FAX number to the facility that can do your testing. MUST be located in the listed States: Indiana, Kentucky, Tennessee, Florida & Kansas, Washington State, where Dr. Cruz is licensed.
- Kentuckiana staff will email the prescription to patients so they can call to schedule their testing. (EMAIL will have the subject: Heart & Arterial Testing Prescription has been sent to the Hospital System you requested).
