Brief Screen for Adolescent Depression

This adolescent depression screen may be used to assess for possible depression and suicide factors. It can assist the clinician in recommending a course of action, as well as to possibly assess the degree of depression symptoms and monitor severity. Please answer all questions as honestly and openly as possible. If you are taking this on behalf of a loved one, please note that all of these questions will be phrased as if that person were taking this screening. If suggested, contact us as soon as you can.

0 – 1 > Mild
