What “Better Blood Flow” Can Mean for Stroke Rehabilitation in Louisville, Kentuckiana: Walking Distance, Hands, and Focus
Regenerative Medicine by Dr. Rafael F. Cruz MD who has practiced medicine for 30+ years and is a physician educator researcher and inventor of Cruz Cric.
Dr. Rafael F. Cruz MD board-certified in Emergency Medicine twice and has practiced for about 10 years. He received 2 years of anesthesia training at the University of Louisville, with an emphasis on procedures that relieve pain. His career then shifted to Integrative, functional, and regenerative medicine, focusing on helping patients improve their health, manage complicated conditions, and reduce the risk of both acute and chronic diseases.
Dr. Cruz is a fellow and founding member of the American Academy of Ozone Therapists. He also received training in IV Vitamin Therapy, IV Chelation and IV Chelation from ACAM (the American College for the Advancement in Medicine). Dr. Cruz also trained at the Riordan Clinic in Wichita, Kansas, which focuses on complementary approaches to care, including IV vitamin C and IV Ozone, used to support cancer patients and those with chronic immune dysfunction like autoimmune Lyme syndrome.
Dr. Cruz, a clinician by profession, is also an educator, a medical writer, a lifelong student and inventor of the “cruzcric”, a pocket-sized emergency surgical airway tool.
Dr. Cruz is the founder and director of Kentuckiana Integrative Medicine, a clinic that combines the best of modern conventional medicine with natural integrative therapies. He is known for his thorough and detailed evaluations. He works collaboratively with primary and specialist care providers. When needed, Dr. Cruz uses image-guided ultrasound and or fluoroscopy to improve injection accuracy for soft-tissue and musculoskeletal conditions, including tendon and rotator cuff tears, and arthritic joint and spinal conditions. The clinic also supports patients with more complicated needs such as neurological [Stroke, Brain injury, Parkinsons, MS, CRPS], Mental Health Addiction [Ketamine Therapy], cardiovascular issues, hormonal optimization, sexual health, sexual dysfunction, stress urinary incontinence and medical weight loss.
Kentuckiana Integrative Medicine has been serving Louisville and the greater Kentuckiana area for over 20 years, providing compassionate care and individualized therapy plans.
Stroke recovery requires more than just a few weeks’ effort. Stroke recovery requires a steady effort of PT and OT, as well as daily home practice. It also takes mental endurance to remain engaged even when progress is slow. We see it every day at Kentuckiana Integrative Medicine with our stroke rehabilitation in Louisville patients. Our approach to recovery is based on a clinical question: What internal factors help the brain and body respond, and what factors might be quietly limiting progress?
We consider brain blood flow and removal of brain inflammation as some of the most critical factors. Many patients report neurological improvements in their daily lives when inflammation is reduced and blood flow is improved. Patients describe feeling lighter, a hand responding more consistently, improved thinking, steady balance, better walking and increased strength and stamina. These are not abstract goals. They’re functional markers that influence independence, self-confidence, and daily quality of life.
Repetition and recovery are the foundations of rehabilitation gains. The nervous system can run out of energy, muscles can fatigue, or cognitive stamina can be low. This can lead to fewer quality repetitions and stalling of progress, even when a person is exerting strong effort. The brain and body can tolerate more activity, recover more quickly between sessions and get more out of every therapy session or home exercise routine when brain inflammation is reduced and vascular system function is improved.
Our approach is integrated. Stroke recovery is not viewed as a neurologic or muscular issue. We structure our care to support the bigger picture of improved neuromuscular function, such as walking distance, arm and leg function, focus and concentration. These impairments often improve when the brain and vascular system are moved in the right directions.

How Blood Flow Affects Stroke Recovery
A stroke is an event that is usually caused by reduced blood flow to the brain, leading to brain inflammation, some brain cell death and reduced brain blood flow. Many people, after the emergency phase, focus on what they can feel and see: weakness, spasticity or balance problems, changes in speech, numbness or pain, mood swings, or brain fog.
The patient’s recovery may be affected by the small and large blood vessels that have experienced blockage, resulting in brain inflammation and damage. If the vascular system is stressed (plaque buildup, impaired circulation, inflammation, endothelial dysfunction), then the brain and other peripheral tissues may not receive optimal oxygen and nutrients. This can show up as:
- Exercise tolerance reduced
- Recovery after physical activity is slower
- Cold extremities and poor peripheral circulation
- Cognitive fatigue and brain fog
- Unexplainable “Plateaus”

What “Better Blood Flow” Can Look Like in Daily Life
Stroke survivors seldom describe their recovery as a lab number; instead, they describe it by functional tasks, such as walking through the grocery store, carrying bags, buttoning shirts, finding words to use in a conversation, or keeping focus in a noisy environment.
When patients want help, they are often referring to functional improvements in areas such as:
- Walking distance and endurance
● Hand control and fine motor consistency
● Walking and balance
● Mental clarity, stamina, and attention
● Recovery time (less “payback”) after an activity
These are the results that people see as restoring their lives.
Walking distance: Why endurance is often the real limiter
After a stroke, walking is more than just strength. It involves timing, coordination and balance. Many survivors are able to walk, but it is their endurance that breaks first.
- Legs start to drag
- The hip muscles spasm
- Balance becomes unstable
- The body stiffens
- Fatigue increases
Some patients say that when circulation is improved and vascular function increases, they can go further before these breakdowns occur. A modest increase in endurance is a huge deal, as it allows for more repetitions in therapy and everyday life.
It’s also about neuroplasticity. Neuroplasticity refers to the brain’s ability to rebuild neural pathways, which, when strengthened with physical therapy, can have a tremendous effect on a patient’s ability to function. The brain can build more pathways if you do more quality repetitions. Working smarter and not harder is one key. Four 10-minute home PT sessions can be more effective than 60 minutes of overexertion and frustration.

Blood Flow and The Brain’s “Battery Life”
Sometimes, stroke patients report they can physically do the rehabilitation work but struggle with mental focus and stamina. Lumosity.com can help greatly as a tool with memory, focus and concentration.
- brain fog
- slow processing
- short attention span
- word-finding difficulty
- fatigue in stimulating environments
The brain’s demand for oxygen and nutrients is high. When vascular health is compromised, especially in patients who also carry a heart disease history or plaque burden, cognitive stamina may be one of the first things to suffer.

Integrative and Regenerative Therapy for The Brain-Heart Connection
Our approach is designed to address those contributors with therapies that support circulation, calm inflammation pathways, and improve the body’s capacity to participate in recovery.
Below are key therapies we offer to support stroke, heart, and vascular conditions.
PSE Perispinal Etanercept Therapy and Post-Stroke Inflammation
Perispinal Etanercept is used as a therapy to support stroke and neurological recovery. Decreasing brain inflammation signalling may help with chronic stroke symptoms that persist after initial brain injury. Our clinic has seen stroke patients recover the function of speech and balance even 15 years after a stroke. Perispinal Etanercept is not FDA-approved for stroke despite a tremendous amount of evidence proving beneficial effects.
When neuroinflammation is part of the therapy approach, patients may notice “rapid” improvements [within minutes] compared to the normal slow rehabilitation timeline. These improvements include clearer thinking, improved movement, improved walking and arm use and reduced pain patterns.
IV Chelation Therapy: Vascular Support and Heavy Metal Reduction
IV Chelation Therapy with EDTA can help support vascular health and bind heavy metals. Chelation goes parallel to cardiovascular and circulation conditions, such as:
- Heart disease
- TIA History
- Peripheral artery disease
- Cognitive decline and “brain fog” symptoms may overlap with vascular problems
NOTE – Chelation is NOT available in Kentucky since it was declared ILLEGAL in KY and three other states. Very few clinics in Indiana offer IV Chelation.
IV Plaquex (IV phosphatidylcholine / IV phospholipids): plaque and vascular resilience
IV Plaquex Therapy is described on our site as using phosphatidylcholine (a phospholipid) to support the body’s response to fatty plaque in vessels. This is often positioned as a complementary vascular-support approach, especially for people thinking long-term about:
- Circulation
- Endurance
- cognitive clarity
- cardiovascular risk factors that contribute to brain health
Many stroke survivors are living with both neurologic deficits and an underlying vascular dysfunction. Improving vascular function can be part of supporting brain function.
Regenerative injections and supportive therapy: removing barriers to rehab participation
Stroke recovery is often complicated by secondary issues: shoulder discomfort, hip pain, back pain, joint strain, and compensation patterns that build over time. Pain and restricted mobility can reduce participation and reduce repetition, slowing progress.
We may include regenerative injections as part of our overall regenerative medicine offerings. In practice, the goal is often to reduce pain-driven limitations and help people move more freely, so they can better engage in the physical therapy recovery work that matters most.

When a patient’s brain function improves, they want functional improvements:
- How they feel and function today (walking distance, hand use, focus)
● How resilient their system is long-term (reducing the conditions that contribute to future vascular events)
Our Stroke Rehabilitation in Louisville is designed to go beyond physical therapy exercises alone. We look at the external and internal environment that influences performance and recovery capacity.
REGRETABLY INSURANCE CANNOT BE ACCEPTED as these therapies are NOT FDA approved, NOT covered by insurance who considers these therapies “experimental” in nature despite the wonderful evidence of tremendous benefits. Insurance pays only for conventional modern medicine. Strategies are considered experimental and not conventional. The approximate cost of the therapy for a full series of Heart IV Therapy is between $4,000 and $8,000, depending on the patient’s condition. IV Plaquex therapy costs $170, and IV chelation therapy costs $140. For most individuals $4,000 to $8,000 is a very reasonable investment in their health, as that is the sum spent on one vacation in many cases. We are very competitive as far as fees, as many clinics charge 2-4x more than we do.
What is meaningful progress?
Stroke recovery doesn’t have to be dramatic. Most often, the most important wins are practical.
- Walking further with less imbalance and more security
- Transferring from chair to bed with greater ease
- Using the affected hand or limb throughout the day
- Conversations that are less mentally exhausting
- Tolerating an activity without a crash of the whole day afterwards
- Moving with more confidence in public places
These are the moments that can change your quality of life.
Stroke survivors often feel a change in their functional abilities when their brain inflammation is reduced. Supporting circulation can make rehab efforts even more effective: walking can be longer, hands can react more consistently, and focus can improve. Our integrative model addresses neuroinflammation and vascular health alongside rehabilitation principles.
Kentuckiana Integrative Medicine believes that stroke is more than just a neurological event. It is also an inflammatory event that can affect your function. Our goal is to create a plan for you that supports your brain, supports your vessels, and supports daily life functionality.
